Trinity Athletics Medical Forms - Parent Portal

Parents must complete the online medical release forms through the Rank One Parent Portal (click link above) prior to a student athlete participating in any organized athletic program. Included on this site are the Medical Release, Concussion Information, and Parental Consent forms.

Athletic Physical Forms

Each student participating in Lower School, Middle School, Junior Varsity, or Varsity sports must complete the Athletic Release Form and have a current physical on file prior to participating in any organized school sporting event.

KHSAA 9th thru 12th Grade Physical

KHSAA MS 5th thru 8th Grade Physical Form

All student athletes participating in an organized sport sponsored by Trinity Christian Academy’s Athletic Department must have a physical on file prior to participating in the athletic event.  This physical must be dated to ensure the date of the physical covers the athletic period.
